This will include blood sampling, recording ECG’s, monitoring prescribed medication, and following guidance for treatment with antipsychotic medication. To support people to develop and achieve the outcomes of their Health Care plan, working with other agencies to ensure the highest standard of patient care Provide a range of health and wellbeing information and support to service users and their families. This will include advice and information relating to healthy eating, exercise, weight management, smoking and substance misuse. To liaise effectively with all members of the multi–disciplinary team and other agencies involved in the care of the individual thereby maintaining and enhancing close professional working relationships. To work with individuals and their families at a variety of venues including patients own homes, hospital bases and a range of community settings. To record and maintain all professional, personal and clinical information of patients in a confidential manner according to the current confidentiality Policy of the Trust and in line with current IT systems and Trust Policies. Support junior staff who escalate concerns raised by patients in their clinics-such as Safeguarding/Medicine Management/Physical health concerns.
