This will include focusing on medicines reconciliation on admission, discharge facilitation, patient counselling, medicines supply and delivering pharmacy services in clozapine and depot clinics in the locality. Extended roles include contributing to the audit and teaching agendas and supporting the coordination and supply of clozapine to community patients, including near patient testing at clozapine monitoring clinics.
