We are looking to recruit an experienced and highly motivated Medicines Management Specialist Pharmacy Technician to join the pharmacy team at CPFT, working alongside a small but friendly, dedicated and experienced team of pharmacy staff.

Based on The Intermediate Care Unit at The City Care Centre in Peterborough, you will be responsible for the provision of the day-to-day clinical medicines management technician service to the wards at The Intermediate Care Unit, and in addition provide support to Trafford ward, in Wisbech, if needed.

This role will include undertaking patient medication histories, medicines reconciliation, assessment of patient’s own medicines and maintaining the system of ‘dispensing for discharge’.

### Main duties of the job

The successful candidate will work closely with the specialist clinical pharmacists and multidisciplinary team to identify patients who have specific medicine management needs and plan to resolve these during their stay. You will promote rehabilitation and independence of patients, by supporting self-administration of medicines and providing medicines information and advice, helping to ensure medicines are used effectively, and improving awareness of medicines and their side effects. You will also support effective planning and organisation of medicines for discharge to enable a seamless transfer of care by liaising with appropriate healthcare professionals both within and outside the Trust.

The ideal candidate will have a flexible approach to needs of the service and have a strong teamwork ethic. They must be well organised and able to prioritise their own work, have good communication skills and be confident working independently, and as a fully integrated member of the multidisciplinary team. This role requires a strong sense of accountability, accompanied by the ability to work under pressure at times.

### Detailed job description and main responsibilities

- Be accountable for developing and maintaining the day to day specialist pharmacy technician services provided to The Intermediate Care Unit, Peterborough and to support the pharmacy service on Trafford ward in Wisbech, if needed in line with medicines policies and standard operating procedures. 2. Support the pharmacist in the provision of the clinical pharmacy service, and recognise when clinical queries need to be referred to a different healthcare professional (e.g. pharmacist, doctor). 3. Undertake medicine history and medicines reconciliation for newly admitted patients, and when necessary, resolve problems by communicating with patients, carers, GPs and other health care professionals and document actions and outcomes accurately. 4. Assess patient’s own medicines for suitability for use. 5. Order medicines from the Trust pharmacy for individual patients for use on the ward and in preparation for discharge. 6. Order stock medicines, maintain stock rotation, including checking expiry dates 7. Label / re-label medicines when required. 8. Induct, train, advise and support nurses and nursing associates on the ward 9. Maintain records of pharmaceutical interventions if required. 10. Liaise with community pharmacies on admission and discharge of patients where appropriate using PharmOutcomes Discharge Medicines Service to provide a seamless medicines pathway into primary care.

The responsibilities of this post require the post-holder to work autonomously as well as a team member. This role may require travel to other locations within the Trust.

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

- Medicines Management Skills or equivalent
