Essential for Band 5 - Experience in a hospital pharmacy, post qualification. Essential for band 5 - Experience of a Patient’s Own Drug scheme. Essential for band 5 - Experience of undertaking patient counselling including high-risk medications such as NOACs in preparation for discharge. Essential for Band 5. - Accredited Medicines Reconciliation (formerly Medicines Management).
