Harbourside Health Centre is seeking an enthusiastic and motivated Practice Pharmacist to join our established, digitally progressive practice on the Swansea Waterfront.

We are a well organised, supportive team that prioritises sustainable workload, collaborative multidisciplinary working and modern systems that allow clinicians to focus on delivering high-quality patient care.

We believe medicines optimisation should be proactive, clinically rewarding and fully integrated within the multidisciplinary team. You'll work alongside experienced GP Partners, nurses and administrative staff in a practice where pharmacists are valued as an integral part of patient care.

### Main duties of the job

Working closely with our GP Partners and wider clinical team, you will contribute to the safe, effective and efficient use of medicines across our practice population.

Responsibilities include:

- Structured Medication Reviews (SMRs)
- Medicines optimisation and polypharmacy reviews
- Medicines reconciliation following hospital discharge
- Long-term condition medication reviews
- Responding to medication queries from patients and clinicians
- Prescription authorisation within agreed competencies
- Clinical audit, prescribing safety and medicines governance
- Supporting QOF, Enhanced Services and Quality Improvement initiatives
- Liaison with community pharmacies and secondary care providers

## Person Specification

### Experience

**Essential**

- Demonstrable post-registration clinical experience.

**Desirable**

- Experience in general practice or primary care.
- Experience undertaking Structured Medication Reviews.
- Experience using EMIS Web.
- Experience managing patients with long-term conditions and polypharmacy.

### Qualifications

**Essential**

- Master of Pharmacy (MPharm) degree (or equivalent qualification recognised in the UK)
- Registered Pharmacist with the General Pharmaceutical Council (GPhC)

**Desirable**

- Independent Prescriber qualification.
