### Job summary

Westminster Living Well PCN is looking for a part-time dietitian who will work alongside members of practice-based and neighbourhood multi-disciplinary teams.

Whether you have 2-3 years experience stepping up into this post or are a seasoned professional moving into primary care for the first time, we are proud to offer a supportive and high-achieving environment. The role would be well-suited to dietitians who have skills and training on conversational approaches to lifestyle and behavioural change in one-to-one consultations and in group settings.

### Main duties of the job

The post-holder will have a high degree of autonomy and will be responsible for the specialist clinical assessment and ongoing dietetic management of adult and some paediatric patients with a number of different clinical conditions.

These will include a complex and wide range of diet-related conditions. You will assess, diagnose, treat,and manage dietetic problems and undifferentiated conditions, and where appropriate, discharge a patient without a medical referral.

- Weight management
- Obesity
- Frailty
- GI conditions
- Nutritional support

Clinical Practice

Assess, diagnose, triage, and manage adult and some paediatric patients,taking responsibility for the management of a caseload

Develop and make use of your full scope of clinical practice, including skills relating to supplementary prescribing and investigation

Manage complex interactions, including working with patients with psychosocial and mental health needs, referring to Health and Wellbeing Coach when appropriate

Deliver programmes of supported patient self-management in ways that facilitate behavioural change, optimise individuals' health, nutrition, fulfilment of personal goals and independence, and minimise the need for pharmacological interventions

Provide care which is tailored to the individual needs

Support patients to set their own goals and be confident in their approach to self-management where appropriate

Evidence, Research and Development

Develop relationships and a collaborative working approach across the PCN supporting the integration of pathways in primary care

Work as part of an MDT in a patient facing role, using your expert knowledge of dietetics, to create stronger links for the PCN dietetic service through clinical leadership, supervisory and evaluation skills

Support research and audit programmes to evaluate and improve the effectiveness of the PCN service

Streamline the pathway of care by providing a responsive service so that patients receive timely access to care

Leadership

Lead, manage and contribute to service delivery

Implement all aspects of effective clinical governance for own practice, including undertaking regular audit and evaluation, supervision and training

Communicate effectively and appropriately, with patients and carers, complex and sensitive information regarding diagnosis, pathology, prognosis and treatment choices supporting personalised care

Facilitating learning

Provide feedback and support to other PCN clinical team members on appropriate referral criteria

Critically reflect on own practice and continue to develop your skills

## Person Specification

### Qualifications

**Essential**

- Registered Dietitian with current Health & Care Professions Council (HCPC) registration
- Degree or equivalent in Dietetics
- Evidence of CPD (adult and paediatric)
- Post-graduate training or equivalent relevant experience within one or more specialist areas of Dietetics
- Minimum 2 years of clinical experience in dietetics (adult and paediatric)
- An up-to-date understanding of Health Education England's Roadmap to Practice

**Desirable**

- Previous clinical experience of first contact triage clinical work; even if in the non-NHS sector or freelance
- Previous experience of working in a GP practice and/or university-affiliated medical provider
- Previous experience of working with frail and elderly patients
- Previous experience of using SystmOne / EMIS

### Key skills and attributes

**Essential**

- Ability to work under pressure in a fast-paced and changing primary care and PCN environments
- Strong clinical problem-solving and analytical skills, including ability to identify when to refer a patient to another service or for diagnostics
- Ability to critically reflect on your own professional developmental needs and is motivated to progress your clinical development (with support)
- Team player with strong communication and interpersonal skills, with an ability to work collaboratively with a wider PCN and practice teams
- Able to follow legal, ethical, professional and organisational
- policies/procedures and codes of conduct
- Office skills including the use of IT/clinical programmes, databases and other modern ways of working and collaborating
