### Job overview

The Patient Safety Midwife provides senior support to the perinatal leadership team and coordinates maternity safety, incident management, learning systems, and regulatory compliance. The postholder ensures high quality investigations, robust governance, compassionate family engagement, and effective dissemination of learning across the maternity service.

Whilst the post holder will be mainly based at the St Helier Hospital site, they are expected to work flexibly across all locations of the Trust. The post holder will work in close partnership with the Patient Safety Midwife based at the Epsom Hospital Site and provide cross cover as required.

### Main duties of the job

The post holder is responsible for:

- Managing incidents raised using the Trust’s incident management system (DatixWeb) including screening, cleansing, ensuring the level of harm allocated is appropriate and allocating incident to the appropriate manager for onward investigation where a formal PSIRF learning response is not indicated.
- Assessing whether a formal learning response may be required and progressing this through the local Incident Review Panels.
- Chairing or contributing to the maternity incident review panel, ensuring robust discussion, learning, and action development.
- Reporting eligible, late miscarriages, stillbirths and neonatal deaths via the national Submit a Perinatal Event Notification (SPEN) portal and ensuring that MBBRACE-UK surveillance information is completed in line with national guidance and timeframes.
- Completing factual questions on the PMRT tool and collating evidence for PMRT reviews.
- Arranging PMRT panel meetings: preparing and circulating agendas, sourcing external panel members, and ensuring documentation is complete.
- Ensuring families are informed of the role of MNSI and where applicable the role of the NHS Resolution Early Notification Scheme (ENS).
- By liasing with the Trust Legal Team, ensuring they are informed of all cases that meet the ENS criteria.

## Person Specification

### Experience

**Essential**

- Knowledge and understanding of clinical governance issues within Maternity
- Evidence of implementing change in practice
- Experience of drafting clinical reports
- Experience of working with the MNVP/wider regional teams

**Desirable**

- Current or recent experience of working in a Patient safety role

### Skills and Knowledge

**Essential**

- Excellent verbal, written and interpersonal communication skills
- Computer skills, with a robust knowledge of working with Excel spreadsheets
- Good presentation skills
- Excellent time management
- Ability to prioritise and meet deadlines
- Ability to analyse problems and implement solutions

**Desirable**

- Knowledge of risk management strategies and PSIRF
- Able to write complex reports

### Qualifications and Education

**Essential**

- Band 7 midwife with UK professional registration or senior Band 6
- First degree or significant equivalent experience

**Desirable**

- Patient Safety/Risk Qualification or equivalent experience of clinical governance
- SEIPS, PSIRF and/or Human Factors training
