### Job overview

The Patient Safety team sits within the Corporate Directorate of Nursing and Quality. The team is required to set the strategic direction for patient safety, to deliver on reactive and proactive aspects of patient safety improvement work and to deliver education and training to enable and support front line staff to keep patients and staff safer. The job purpose for the Patient Safety Coordinator is to support the patient team in the delivery and embedding of a culture of continuous learning and quality improvement within the Trust.

In this role you will provide high quality administration support and participate in the day-to-day delivery and development of the key workstreams to ensure service improvements and the key objectives of the patient safety strategy. The post holder will help the Head of Patient Safety with the organisation and coordination of work within the team to ensure work is tracked and timescales are met. The objectives of the role will be regularly reviewed to align to organisational priorities.

The post holder will need to be flexible in providing administrative support to the team. A driver would be desirable.

### Main duties of the job

- Provide high quality project, service, initiative and administrative support including participating in information analysis.
- Prepare clear and accurate meeting minutes including those involving confidential or sensitive subject matter.
- Handle confidential and potentially distressing information with discretion and professionalism.
- To act with integrity and be responsible and accountable for own workload and develop good working relationships with clinical and operational staff.
- Organise and administrate the patient safety meetings, safety discussions and after-action reviews (AAR’s) - to include the production of invites, agenda and minutes to update members.
- To communicate effectively with colleagues at all levels.
- To support the tracking and progress of all reviews and investigations led by the team.
- Support the compliance with all statutory reporting requirementsParticipate in continuously promoting a culture of learning, patient safety and quality improvement.
- To deliver and request information to/from external providers and commissioners.
- To update information and upload documents on the Trusts incident reporting system (InPhase).
- Assist in the planning and administration relating to day to day management of reviews, investigations, reports and improvement workstreams and initiatives.
- Work within the developing Safety II culture, linking both excellence and learning from patient safety events.
- Provide administrative support for the InPhase team when required.

### Applicant requirements

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.

## Person Specification

### Experience

**Essential**

- At least one-year experience of working in governance or patient safety within healthcare.
- Significant administrative or relevant experience including initiating and maintaining office systems.
- The use of Microsoft Office Packages e.g. Word, Excel, PowerPoint.
- Demonstrable experience in dealing with sensitive and confidential information.
- Minute taking or note taking skills

**Desirable**

- Experience of working for the NHS.
- Experience of working in a mental health trust.
- Working with multi-disciplinary teams in a healthcare environment.
- The use of clinical systems and databases.
- Using the Being Fair Tool

### Knowledge and Skills

**Essential**

- Record keeping and case tracking skills.
- Knowledge of an incident reporting system, StEIS, LFPSE
- Ability to effectively analyse and interpret data.
- Ability to produce well drafted emails and communications.
- Personal resilience in response to exposure to sensitive and some distressing situations
- To respond positively to feedback and to adopt a personal continuous learning approach.
- Ability to work under pressure in a busy working environment and able to multi-task.

### Training, Qualifications and Registration

**Essential**

- Educated to NVQ 3 level in a relevant subject or equivalent level of qualification or significant equivalent previous proven experience

**Desirable**

- Educated to degree level or equivalent in academic study or be willing to work towards.
- Additional specialist training in patient safety incident investigation, clinical risk management or relevant field (e.g. Healthcare Safety Investigation Branch Level 2/3 patient safety investigation training or equivalent).
- SEIPS framework or equivalent training.
- Recognised training in advanced use of Microsoft Office Packages, Excel and Word
