An exciting opportunity has arisen within the Lung Cancer Nursing Team for a permanent, part-time, Patient Pathway Co-ordinator.

The role is for 28.5 hours per week.

### Main duties of the job

You will be required to work closely with clinicians, specialist nurses, secretaries and other members of the Multi-Disciplinary Teams (MDT) and associated departments, to ensure that patients on a cancer pathway are proactively managed. You will be required to coordinate the patient journey from referral, through diagnostics and treatment, highlighting any potential delays in the pathway promptly.

Candidates must have excellent organisational, communication, interpersonal skills and possess the ability to have difficult conversations in challenging environments.

Experience of working within a Healthcare setting is essential for this role alongside an in depth knowledge of National Cancer Waiting Times targets.

### Detailed job description and main responsibilities

The post holder will be responsible for tracking patients who have been urgently referred to the Trust from General Practitioners (GP) who are suspected at referral of having Cancer. Patients who have been upgraded onto a 62 day pathway or have entered the system via a Screening pathway. Ensuring these patients are listed on the SCR IT monitoring system.

The post holder will be responsible for tracking and monitoring all referrals against national cancer targets.

To accurately collect and record cancer pathway related data in the SCR cancer tracking system.

To extract information from other sources and Trust IT systems (i.e. Pathology, Radiology, Endoscopy, iPM etc.) to support the accurate, detailed recording and tracking of cancer pathways for patients on SCR.

To work closely with MDT Co-ordinators and PPC teams to ensure that data collected on each patient on SCR is accurate and tracked on a daily basis.

### Skills and Abilities

**Essential**

- Good attention to detail and methodical in approach
- Excellent interpersonal and communication skills and the ability to motivate, influence, inspire and encourage different and new ways of working
- Able to prioritise and work on own initiative and as part of a team
- Good time management and ability to work under pressure

### Experience and Knowledge

**Essential**

- Previous experience working as an MDT Co-ordinator or Patient Pathway Co-ordinator
- Experience of using computerised data systems

**Desirable**

- Knowledge of medical, anatomical and oncological terminology
- An understanding of the current national issues affecting cancer
- Understanding of patient pathways

### Education and Qualifications

**Essential**

- Good general education with English and Maths to GCSE standard or equivalent
- Evidence of IT skills and Healthcare information systems - knowledge of working with MS office, especially Excel and iPM or equivalent
