An exciting opportunity has arisen for proactive and friendly person to join a well established team at the Royal Hampshire County Hospital.  We are looking to recruit a motivated individual who possesses excellent communication skills, and can work well under pressure to co-ordinate discharges with families and the medical team.  The team has many disciplines and is forever growing with passionate people providing high standards of care to patients   Recent developments have led to a stronger focus on providing Safe and Compassionate care to our patients and this has increased opportunities available to learn, develop and expand your career.   So while we are developing, you can too - within a professionally challenging yet exciting environment

### Main duties of the job

- Gather and be aware of information given and decisions made by the multi-disciplinary team
- Clearly present the patient’s point of view to others, including promoting and protecting the interests and dignity of patients
- Communicate sensitively, confidentially and with empathy to meet the wide ranging physical and emotional needs of patients and their carers and families
- Communicate with patients, carers, relatives and professionals at all levels involved in the discharge planning process
- Communicate with Local Authority care managers to ensure consistent approach and joint working in the interests of patients
- Provide accurate, timely and relevant handover information using electronic systems where available
- Record information about patients in a safe, accurate and consistent manner using appropriate tools and techniques, including electronic and written formats as close to point of care as possible
- Maintain confidentiality at all times, as required by legislation and trust policy
