### Job summary

In this key role, you will coordinate the patient pathway from the point of referral, working closely with families, doctors, ward teams and the discharge coordinators to ensure clear communication and seamless transitions throughout care. Key Responsibilities Coordinate the patient journey from point of referral Communicate effectively with patients, families and multidisciplinary teams Maintain and track patient information using EMIS and Excel Support efficient admissions, transfers and discharges Ensure every patient and family experience is as smooth and supportive as possible Uphold and reflect the values of the hospice in all interactions
