Parkinson's Clinical Nurse Specialist- Permanent

The post, a Clinical Specialist Nurse for Parkinson’s Disease (PD), is part of the Community Neurology Team, in Dudley Rehabilitation Service (DRS), which is a specialist multidisciplinary team providing advice, rehabilitation, treatment and management of patients with neurological conditions across the Dudley Borough.

The post holder will work closely with the acute PD CNS, the elderly care and neurology consultants and the Neurology Specialist Pharmacist based at Russell’s Hall Hospital, as well as the Long term conditions team based in the Dudley Clinical Hub to plan, co-ordinate and deliver care for patients to try to keep them safe and as independent as possible in the community. The post holder will liaise with GPs and Community Partnership Teams in their PCN, attending meeting as necessary; as well as attending the PD multidisciplinary team meetings, and the Community Neurology pathway MDT meetings.

### Main duties of the job

- To manage a caseload of PD patients who need ongoing support, assessment and treatment within the community.
- To have a specialist understanding of medications used to treat PD, and common complications, understanding possible side effects. To educate patients on the various drugs available as necessary and appropriate.
- To prescribe treatments within nurse, non-medical prescribing (NMP) constraints.
- To undertake nurse led clinics for PD patients.
- To have responsibility for and contribute to PD education for staff and patients
- To undertake comprehensive assessments of patients, including those with a complex presentation, using investigative and analytical skills, and to formulate individualized management and treatment plans, using clinical reasoning, and utilizing a wide range of treatment skills and case management skills.

## Person Specification

### Knoweldge

**Essential**

- Specialist knowledge of Parkinson’s Disease and its treatments or willingness to learn.
- Able to carry out self-directed learning
- Willing and able to carryout CPD to keep up to date with clinical skills and knowledge

### Experience

**Essential**

- Evidence of relevant and extensive post-graduate experience
- Experience of carrying out audit
- Experience of working with patients with a neurological or long-term condition
- Experience of facilitating service development

**Desirable**

- Experience of working in the community
- Experience of working with PD patients

### Qualifications

**Essential**

- Diploma / degree in Nursing
- Registered with the Nursing and Midwifery Council
- MSc in relevant clinical area or equivalent post-graduate courses / experience
- Evidence of extensive, relevant post graduate training
- V300 Non- medical prescribing qualification, or, willing and able to complete it
