### Job overview

We are looking for an enthusiastic postholder to work alongside a multidisciplinary team, to contribute to the provision of the highest standards of patient care in Orthopaedic paediatric outpatient clinic. Support patients and careers through their care journey, providing the patient with a single named point of contact who will coordinate their care.

To co-ordinate the patient journey from referral, through diagnostics, treatment planning and treatment, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process.

To highlight relevant patients to the clinical team for discussion at the Multi-Disciplinary Meeting (MDT).

To work closely with members of the orthopaedics, spinal and paediatric team.

### Main duties of the job

- Coordinate and manage patient pathways from referral through to discharge, ensuring patients progress through their care within agreed waiting time targets.
- Manage outpatient clinic schedules, appointments, waiting lists and clinic capacity to maximise service efficiency in conjunction with administration staff
- Work closely with consultants, nurse practitioners and the multidisciplinary team to ensure investigations, follow-up appointments and treatments are arranged and completed.
- Act as a central point of contact for patients, relatives, healthcare professionals and external organizations, providing information and support throughout the patient's journey.
- Maintain accurate patient records, update demographic information and ensure clinic outcomes are recorded correctly.
- Monitor patient pathways, identify delays or backlogs and take action to resolve issues, escalating where necessary.
- Support the delivery of outpatient clinics by preparing clinical areas and equipment, assisting with procedures, chaperoning patients and responding appropriately in emergency situations.
- Arrange additional support services such as transport, translation and specialist services to meet individual patient needs.
- Contribute to service improvement initiatives, audits, quality programmes and the development of patient pathways.
- Ensure confidentiality, infection prevention, health and safety, and Trust policies are adhered to at all times.
