### Job overview

An exciting opportunity has arisen for a Band 6 Paediatric Clinical Nurse Specialist within the field of Haemoglobinopathies.

The post holder will work as part of a comprehensive multidisciplinary team as part of Sickle Cell and Thalassaemia Services with Homerton NHS.  The service is integrated, and the post holder will be required to work across the community and the acute settings, including attending meetings at the Royal London Hospital as well as home visits to meet service need and patient demands.

You will be required to work autonomously as well as part of the team.  You will be expected to engage with patients and their families on a regular basis, planning, initiating and implementing care as required.  You should possess excellent communication skills and the confidence to liaise with other health professionals on behalf of the patient.

The successful candidate will provide specialist nursing care, education and support for individuals with Sickle Cell & Thalassaemia.

The Homerton serves a significant Sickle Cell and Thalassaemia patient population and treats patients referred from inside and outside the City and Hackney area. Therefore, it is essential that post holder has a good knowledge and understanding of the implications and nature of the disease and its processes.

### Main duties of the job

To work with the Nurse Consultant and members of the Sickle cell & Thalassaemia team, including the wider multi-disciplinary team to provide comprehensive care, for people with Sickle cell & Thalassaemia Disorders. It will be essential to work across the community and the hospital setting to improve standards and equity of care whilst promoting an integrated care pathway, following up on newborn screening results and planning and implementing care plans.

The post holder will be expected to comply with National and Local standards and guidelines in the management of Adults and Children with Sickle Cell & Thalassaemia disorders.  Active health promotion and outreach work will promote the work of the sickle cell team and raise awareness in the local community.

- Effective communication within the team is essential as well as effective liaison with other agencies such as social care, community nursing teams and safeguarding.
- Intensive caseload management, provide professional expertise & care for paediatric patients receiving treatments or interventions in the community or hospital setting.
- Co-ordinate and develop clinical practice in the community for paediatric patients, such as community review clinics.
- Provide high quality information, education on haemoglobinopathies to schools, colleges and other services as required.
- Cannulation & Venepuncture in accordance with local policy.
- Develop and sustain effective relationships with public health teams and General Practitioners.
