The post holder will work as part of a comprehensive multidisciplinary team as part of Sickle Cell and Thalassaemia Services with Homerton NHS.  The service is integrated, and the post holder will be required to work across the community and the acute settings, including attending meetings at the Royal London Hospital as well as home visits to meet service need and patient demands. You will be required to work autonomously as well as part of the team.  You will be expected to engage with patients and their families on a regular basis, planning, initiating and implementing care as required.  You should possess excellent communication skills and the confidence to liaise with other health professionals on behalf of the patient. The successful candidate will provide specialist nursing care, education and support for individuals with Sickle Cell & Thalassaemia. The Homerton serves a significant Sickle Cell and Thalassaemia patient population and treats patients referred from inside and outside the City and Hackney area. It will be essential to work across the community and the hospital setting to improve standards and equity of care whilst promoting an integrated care pathway, following up on newborn screening results and planning and implementing care plans.
