QVH places a strong emphasis on the availability of timely, accurate and relevant clinically coded information to assist all staff in the planning and delivery of the best possible healthcare. Clinical Coding plays a crucial role in the management of the Trust as it directly influences income through the ‘Payment by Results’ process. It also forms the  basis for clinical audit, clinical governance reporting, benchmarking, clinical performance indicators, information governance and medical research.

The Clinical Coding team:

Deliver accurate, timely and complete clinically coded data  Foster a culture of continuous improvement and service development by  ensuring all team members have up to date training and that the department participates in regular audit where feedback actions are embedded in everyday practice  and develops strong working relations with Clinicians

### Main duties of the job

- Deliver accurate, timely and complete clinically coded data
- Abstract, analyse and enter correctly coded data into PAS (Patient Administration System) within agreed timescales.
- Locate and collate all appropriate source material, for example, missing medical record and communicate complex coding queries.
- Build and maintain knowledge of coding procedures and conventions, medical terminology and anatomical and physiological terms.
- Be able in the use of hospital systems for the input of clinical data; paying particular attention to accuracy, timeliness and confidentiality.
- Have knowledge of the importance of clinical coding and its uses

### Detailed job description and main responsibilities

- Abstract, analyse and enter correctly coded data into PAS (Patient Administration System) within agreed timescales.

- Build and maintain knowledge of coding procedures and conventions, medical terminology and anatomical and physiological terms.
- Be able in the use of hospital systems for the input of clinical data; paying particular attention to accuracy, timeliness and confidentiality.
- Have knowledge of the importance of clinical coding and its uses and an understanding of data quality requirements.
- Abstract and analyse patient clinical records.
- Access electronic software systems to extract information  Assist clinical teams with queries
- Share and demonstrate processes to team members as required.
- Organise and prioritise own workload; escalating any issues to the Clinical Coding Manager.
- Take an active role in the development and implementation of local policies and procedures.
- Personal and Professional Development
- Participate in the Trust’s annual appraisal system designed to identify objectives and personal development opportunities.
- Participate in the Trust’s annual pay progression review (if applicable).
- Ensure that all mandatory training is up to date
- The post holder will be responsible for continually maintaining their working knowledge of local practices, policies and procedures and highlight ongoing training and development needs to their manager.

### Qualifications

- GCSE level or able to demonstrate knowledge within this field through practical experience

- Prolonged periods of VDU use
- Frequent requirement for prolonged concentration
- Frequent exposure to medical records which can , on occasion, contain disturbing images and information about abuse and terminally ill patients
