An exciting opportunity has arisen to join our specialist stroke early supported discharge team . the team covers Merthyr Cynon rct and Bridgend area.

This role involves working with patients who have experienced a stroke and require further rehabilitation within their own homes to maximise their independence.  the post will involve completing assessment and interventions within the patient own property and local community with in- reach opportunities to our acute and rehabilitation sites.

### Main duties of the job

As an Occupational Therapy Assistant Practitioner you will have the opportunity to work along side registered Occupational Therapists and manage a delegated caseload of patients. You will be able to support the assessment, treatment of people  following a Stroke  who may have a range of functional difficulties within the bounds of the technician role.  Patient management will include responsibility for the delivery of care including liaison with patients, family, members of the MDT and external agencies.

You will need to have good time management, prioritisation skills, and flexibility to the service. In addition you will need to have  the ability to communicate effectively both verbally and in writing.  The post holder will be predominately working within the community and relevant hospital bases as required.

You will be an integral part of the team and have a key role in its delivery of high quality interventions. You will have support from the Senior clinicians and the Clinical Lead, with access to supervision and in-service training opportunities.  You will be required to achieve level 3 / level 4 Agored qualification in Occupational Therapy Support.
