The post is a key part of the OT discharge service supporting registered staff with the assessment and rehabilitation This role includes :- - Assessment of trauma and elective patients who have been referred Occupational Therapy - Taking equipment out to patients’ homes and fitting items (training will be given) - Setting up / providing follow up visits for prescribed equipment, - maintenance of stock, - Liaison with outside agencies and patients’ families, - Teaching patients and their families how to use and fit the equipment - Undertaking falls assessments The post is based at Wexham Park Hospital but applicants must be able to travel between all Frimley Health's hospitals and patient's homes.
