An opportunity has arisen to join our committed and caring inpatient team at Ludlow hospital. We are seeking a motivated and skilled Occupational Therapist to join our Locality Therapy Team. This position will be based at Ludlow Community Hospital. The successful individual will join an enthusiastic and supportive multi-disciplinary team of Therapists, Nurses, GPs, ACPs and support staff in providing high quality care, rehabilitation, and discharge planning. The post holders’ main responsibilities will be providing Occupational Therapy assessment and rehabilitation to patients who have a wide range of conditions and complex needs, with an aim of maximising independence and supporting a safe hospital discharge. The successful individual must have an interest and experience in rehabilitation within the Community setting and be able to work as an autonomous practitioner, managing their own caseload. There is an opportunity to outreach into beautiful South West Shropshire, to provide Occupational Therapy interventions to patients in their own home. We have a strong commitment to clinical supervision and Continuing Professional Development (CPD), offering a supportive and proactive environment with opportunities for supervision, training, and peer support, both locally and County wide. This post will be involved in supervision of junior staff, assistants and students. ### Main duties of the job

- To provide a high quality seamless Occupational Therapy service from Acute or Community Hospital to Community, responsive to the needs of the local population as part of a cohesive multidisciplinary team.
- To work as a key member of the multidisciplinary team (MDT) that is responsive to the needs of the local population, working flexibly to respond to the changing needs of the patient.
- To provide a key role in facilitating the seamless transition from community hospital to patient’s own home and transferring the care of the patient to other appropriate services to maximise the patient's independence and quality of life.
- To provide supervision, facilitate the personal and professional development of junior members of staff, enhancing their skills and knowledge and where appropriate line management.

### Experience

- Significant Post graduate experience in a variety of clinical settings with experience in a community setting and inpatient setting
- Experience of working in multi-disciplinary / multi-agency teams.

### Skills and Knowledge

- Ongoing awareness of recent developments in practice and research including NSF’s/NICE guidelines.
- Understanding of clinical governance and risk management and their implications for the service.
- Excellent time management skills.
- Excellent interpersonal and communication skills.

### Qualifications and Training

**Essential**

- Diploma/degree in Occupational Therapy
- Current registration with HCPC
- Evidence of core rotations
- Evidence of specialist post graduate training and education with older adults.
