An exciting opportunity has arisen for a full time / part time band 5 OT to join our newly developing Pennine Community Mental Health Rehabilitation Team.

LSCFT Specialist Mental Health Rehabilitation is currently undergoing transformation across inpatient and community services in order to implement a robust pathway for people with complex mental health needs in the least restrictive and safest way. There is a need to develop the range of service provision that will enable this.   The aim of the transformation is to provide a service which can offer a whole pathway of support locally, identified in both national plans and clinical guidance, which would include an inpatient and community service offer that is matched against the identified need of LSCFT population.

This role is to deliver an intensive rehabilitation to service users with complex needs through occupational therapy assessment and intervention and complete devised individualised rehabilitation plans to meet those needs. You will have excellent communication skills, sound clinical reasoning and a strong understanding of the occupational therapy role within recovery.

### Main duties of the job

The post holder will work as part of a specialist mental health rehabilitation team that provides specialist assessment, treatment, and delivery of evidence based occupational therapy interventions, and support to enable the recovery of people whose complex needs cannot be met by general adult mental health services.  This service aims to work with people to help them acquire or regain the skills and confidence to live successfully in the community. They focus on addressing and minimising the symptoms and functional impairment that people may have, with an emphasis on achieving as much individual autonomy and independence as possible.

The post holder will provide hospital in-reach and community-based support to people with severe mental health, significant comorbidities and complex social needs who need intensive community support, supported accommodation or inpatient rehabilitation.
