### Job summary

Leeds Community Healthcare is recruiting Band 6 Senior Occupational Therapists to join the Active Recovery Service or Neighbourhood Therapy Teams. Both services deliver high-quality, person-centred rehabilitation, supporting adults to regain independence and remain safely in their homes and communities.

As a Senior Occupational Therapist, you will support adults following illness, injury, hospital admission or functional decline. You will complete assessments, develop personalised rehabilitation plans and deliver evidence-based interventions to promote recovery, independence and quality of life.

The Active Recovery Service is a partnership with Leeds City Council, providing integrated intermediate care.

It delivers short-term, goal-focused rehabilitation for people recovering from trauma, surgery, neurological conditions, frailty and long-term conditions, using a strengths-based approach to reduce ongoing care needs.

Neighbourhood Therapy Teams provide community-based rehabilitation across Leeds, supporting planned and urgent care. Therapists work with individuals experiencing falls, mobility issues and complex needs, helping prevent hospital admission and support recovery at home.

You will manage a varied caseload, deliver rehabilitation, support discharge and work with multidisciplinary teams. The role also includes supervision, service development and ongoing professional development.

Assess patients holistically following illness, injury, hospital admission, or functional decline

Develop and deliver personalised rehabilitation plans to support recovery and independence

Provide evidence-based interventions to improve function, mobility, and quality of life

Manage a varied caseload across community or intermediate care settings

Support hospital discharge and help prevent unnecessary hospital admissions

Carry out falls prevention work and promote safe living in the community

Complete environmental assessments and recommend/provide appropriate equipment

Work collaboratively with multidisciplinary teams to plan and coordinate care

Contribute to integrated care pathways within community and intermediate care services

Supervise students and support staff Participate in service development and quality improvement initiatives

Engage in ongoing professional development and clinical supervision
