Both services deliver high-quality, person-centred rehabilitation, supporting adults to regain independence and remain safely in their homes and communities. As a Senior Occupational Therapist, you will support adults following illness, injury, hospital admission or functional decline. You will complete assessments, develop personalised rehabilitation plans and deliver evidence-based interventions to promote recovery, independence and quality of life. The Active Recovery Service is a partnership with Leeds City Council, providing integrated intermediate care. It delivers short-term, goal-focused rehabilitation for people recovering from trauma, surgery, neurological conditions, frailty and long-term conditions, using a strengths-based approach to reduce ongoing care needs. Neighbourhood Therapy Teams provide community-based rehabilitation across Leeds, supporting planned and urgent care. Therapists work with individuals experiencing falls, mobility issues and complex needs, helping prevent hospital admission and support recovery at home. You will manage a varied caseload, deliver rehabilitation, support discharge and work with multidisciplinary teams.
