### Job summary

An exciting opportunity has arisen for a Band 5 Occupational Therapist to join the Short Term Care Beds Service on a fixed-term basis or secondment. The service provides short-term rehabilitation for individuals who no longer require acute hospital care but are not yet able to be safely supported at home

We are looking for an enthusiastic and motivated OT who is passionate about delivering high-quality, person-centred care and supporting adults to maximise their independence and achieve their recovery goals

You will work as part of a supportive multidisciplinary team, contributing to the assessment, treatment, and rehabilitation of patients with a range of physical, cognitive, & functional needs

You will have the opportunity to develop your clinical skills while supporting patients through their recovery journey and preparing them for a safe discharge home or to an appropriate care setting

You will receive support from experienced senior therapists and benefit from regular supervision, training, and professional development opportunities to help you grow within your role

This is also an excellent opportunity for a newly qualified or developing Occupational Therapist looking to gain experience in rehabilitation and intermediate care services, while making a meaningful difference to patients' lives

The role offers a varied & rewarding caseload, including supporting patients with long-term conditions, falls prevention & rehabilitation following illness or hospital admission

### Main duties of the job

The post provides an excellent opportunity to develop a broad range of clinical skills and experience across community rehabilitation services

You will work with patients in a holistic and person-centred way, supporting them to maximise their independence, improve their quality of life, and achieve their individual goals. Using health coaching and self-management approaches, you will empower patients to take an active role in their recovery and ongoing wellbeing

As part of a supportive multidisciplinary team, you will work closely with experienced Occupational Therapists, Physiotherapists, Nurses, Rehabilitation Assistants and other healthcare professionals

You will benefit from regular supervision, professional development opportunities, and the shared expertise of colleagues while contributing to high-quality patient care

You will be responsible for delivering and facilitating rehabilitation programmes, undertaking assessments, developing treatment plans, implementing interventions, and evaluating outcomes. Working in partnership with patients, carers and families, you will use strengths-based approaches to promote choice, involvement and independence

The role will support you to develop your clinical reasoning, organisational & communication skills while gaining experience in discharge planning, rehabilitation, falls management, and long-term condition management within a community setting
