## Job Content

### Job overview

We have an exciting opportunity for an Occupational Therapist to join our Integrated Community Team (ICT) based in Forest & Tewkesbury, Newent, Staunton Locality. If you are a newly qualified OT or an OT who is keen to work within our Home first and Reablement service within community health services this could be the job for you.

You will be part of a multi-disciplinary team working within peoples homes, with patients who have a variety of health conditions, including palliative care, complex neurology, long-term conditions, stroke, and frailty.

The ICT consists of Occupational Therapists, Community Nurses, Physiotherapists, and a Home First/Reablement service. We work closely with a range of teams from within our own organisation and key local system partners including, Adult Social Care, GPs, Residential homes, Acute hospitals, Community hospitals, housing and voluntary organisations.

The ethos of the team is a rehabilitation, person-led approach to recovery and self-management, to ensure we offer care close to home, avoiding unnecessary admissions to bed based services. Our philosophy is that supported self-management is key to personalised and positive outcomes for people.

This role is offered on a permanent basis. The hours are full time 37.5 hours per week.

### Main duties of the job

We are a Technologically SMART Trust, offering a ‘Mobile office’ model providing you with all the equipment needed to carry out your role in comfort whilst on the road.  SMART working results in staff safety due to live system and instant access to patient notes and past interactions whilst on the move.

To excel in this role, you will require excellent communication skills, the courage to make clinical decisions and a willingness to develop your potential. You will require current HCPC registration, post-registration experience is helpful but not essential and we have an excellent preceptorship programme for newly qualified Occupational Therapists.

In all aspects of our interventions we promote personalised care, working with our patients to identify what matters to them, promoting independence and enabling them to achieve their goals.

This post requires the post holder to be able to undertake all statutory and mandatory training and post holders must be able to undertake physical interventions. Fitness for physical intervention will be assessed by Occupational Health.

A valid UK driving licence plus the use of own transport for business purposes is essential. For qualifying staff the Trust has a Lease Car Scheme in place and also offers a popular Salary Sacrifice Car Scheme with our partner Tusker.

The qualification, training & experience requirements for the role are underlined in the Job Description/Person Specification.

### Detailed job description and main responsibilities
