Are you an experienced Occupational Therapist who is passionate about driving change in older adult care? Are you looking for a role that gives you the opportunity make change in a supportive, flexible environment?

We are looking for an experienced Occupational Therapist to join our Care Home Support Team on either a full or part time basis for a 12 month FTC.

This is a unique opportunity to use your clinical expertise to train and develop care home staff across East Berkshire, working with the Integrated Care Board, Care Homes and Local Authorities to support delivery of quality care of residents.

Unlike your normal clinical role, your main task will include quality improvement, meeting constant needs for new training, signposting to relevant services and working with care homes to identify gaps in services and how to manage these.

You will work autonomously managing your time between sites, training and virtual support from home however the service is made up by a complete MDT of both physical and mental health professionals so there is plenty of support available.

### Main duties of the job

- To work in partnership with care homes to develop pro-active training/educating advisory service that aims to improve the health, well-being, and independence of all care home residents.

- Identify the specific skills and training requirements for each care home and deliver and record a tailored training programme

- Attend and contribute to clinical/care homes MDT and cluster meetings where appropriate.

- Share expert knowledge and skills with the care homes to enhance their skills.

- To independently manage a small caseload of complex cases relating to mobility, pressure, and postural care and often in combination with communication, challenging, cognitive and/or perceptual difficulties.

- Formulate individualised action plans based on the assessment findings and clinical reasoning.

- Work as a senior practitioner and be professionally and legally responsible for all aspects of your own work.

- Complete support visits to care homes and offer advice and support

- Continue working alongside local authorities to support those homes rated red or amber and those with residents who have frequent hospital admissions.

- Signpost and support with making direct referrals on to other community services or professions as required

## Person Specification

### Previous Experience

**Essential**

- Post registration experience
- Specialist knowledge acquired through training or working with older people with complex health needs
- Experience of working in a multi-disciplinary team and multi-agency environment

**Desirable**

- Experience of working with care homes
- Experience of working with patients with a postural deviation
- Experience of teaching others
- Evidence of participating in clinical audits in the workplace

### Additional Requirements

**Essential**

- Ability to travel to multiple sites efficiently
- Full UK driving licence
- The daily use of a car
