This role is within the Inpatient Rehabilitation setting where you will work within a multidisciplinary team to rehabilitate patients in an inpatient rehabilitation ward setting.

The role is based at City Care Centre, Intermediate Care Unit and you will work closely with your colleagues in other localities and across the system that provide Intermediate Care for the population of Cambridgeshire and Peterborough.

### Main duties of the job

The Intermediate Care Unit Inpatient Rehabilitation team strive to deliver outstanding patient-focused specialist care and therapy which collaboratively optimises a person's potential to achieve their individual goals.

As part of the role, you will:

- provide assessment and occupational therapy interventions to patients admitted for rehabilitation from acute hospitals and from the community.
- implement therapy plans as well as delegating to therapy assistants and integrated care workers.
- develop your skills in sub-acute rehabilitation and discharge planning working with ward based MDT and colleagues from other pathways, social care and the voluntary sector.

. work autonomously in a variety of settings to include travelling across the community completing access visits where needed.

- To be professionally and legally responsible and accountable for all aspects of your own work, including the management of patients in your care.
- To accept responsibility for ensuring the effective selection, use and fitting of equipment.
- To independently plan and carry out assessments in a variety of settings.
- To take responsibility for appropriate delegation of tasks to therapy assistants.

You must hold UK HCPC registration for this post.
