This is an exciting opportunity to be part of a new, community-based rehabilitation service, supporting people with complex mental health needs to develop independence, improve daily living skills and achieve meaningful recovery. You will work under the supervision of an Occupational Therapist to deliver practical, person-centred interventions with individuals in their homes and community settings. The service focuses on functional recovery, helping individuals build the practical skills needed for independent living, including self-care, managing routines and engaging in meaningful activities. The team works using an assertive outreach approach, delivering support in people's homes and community settings.
