We have an exciting opportunity for two Band 5 Dietitians to join our welcoming Community Dietetics Team—one full-time permanent post and one part-time fixed-term post. Working within our Nutrition Support service, you'll help people at risk of malnutrition achieve their nutrition and hydration goals through evidence-based, person-centred care. Using a food-first approach alongside the appropriate use of oral nutritional supplements, you'll work closely with service users, families, carers and multidisciplinary colleagues to improve health and wellbeing. Our hybrid team provides services across Hammersmith & Fulham, Westminster and Kensington & Chelsea, with travel across these boroughs as required. We meet virtually each week and in person monthly, creating a collaborative and supportive working environment. The service includes three specialist workstreams: Nutrition Support, Gastroenterology and Home Enteral Feeding, offering excellent opportunities to broaden your skills and experience. Flexible working options and reasonable adjustments will be considered wherever possible. ### Main duties of the job As a Band 5 Dietitian, you will provide high-quality, evidence-based nutrition support to adults at risk of malnutrition, helping them achieve their nutrition and hydration goals. Your responsibilities will include: - Undertaking dietetic assessments and developing personalised treatment plans through face-to-face, telephone, video consultations and home visits where appropriate. - Managing a supervised caseload, delivering safe, effective and person-centred care in line with professional standards. - Working closely with service users, families, carers and care providers to support positive nutritional outcomes. - Planning and prioritising your clinical and administrative workload, maintaining accurate records and activity data. - Collaborating with multidisciplinary teams across primary, community and secondary care to ensure coordinated care. - Building effective relationships with GPs, district nursing teams, care providers and other partner organisations to support timely referrals. - Delivering education and advice to colleagues and partner organisations where appropriate. - Contributing to service development, audit, quality improvement and your own continuing professional development.
