As a Band 6 Team Leader, you will manage a diverse caseload, delivering and coordinating personalised care across a multidisciplinary team aligned to Primary Care Networks. You’ll triage and allocate referrals, lead on holistic assessments, and ensure safe, effective care delivery—always focused on preventing hospital admissions and promoting independence. You’ll provide expert clinical care including palliative and end-of-life support, IV therapy, wound care, and bladder and bowel interventions.
