Do you have a passion for community nursing and are looking to develop your knowledge and skills within this field. If so, we currently have a Band 3 vacancy within our Community Hospitals in CTMUHB, where free car parking is available.

The successful candidate will work as part of a nursing team of qualified nurses and health care support workers on Ward C3  to provide nursing support to adults with rehabilitation needs and to support complex discharge planning, ensuring that patients are discharged to the most appropriate destination.

### Main duties of the job

- Work as part of the multidisciplinary team to deliver care to patients
- Undertake personal care activities and clinical care duties with minimal support and supervision as delegated by a registered healthcare professional in accordance with the HEIW All Wales Guidelines for Delegation
- Ensure the registered healthcare professionals are regularly updated on the patients/client’s condition and progress
- Ensure the working environment is safe and hygienic following infection prevention and control policies and processes
- Contribute to the teaching and supervision of students and junior staff
- Deliver the role in accordance with Health Board/Trust Values and Code of Conduct for HCSWs in Wales.

This doesn’t mean essential;  whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and  selecting candidates. The Health Board currently operate a number of different shift systems. The most common are a 12.5 hour shift, inclusive of either one 30 minute or two 30-minute unpaid breaks. These shift patterns are subject to review and the Health Board reserves the right to change working arrangements. ### Knowledge

- Level 3 Qualification in Health Care Related subject or Equivalent experience

**Desirable**

- Previous experience in hospital setting
