An exciting opportunity has arisen for an experienced and committed Nursing Support Worker to join the Carmarthenshire Community Nursing Service and Continuing Care Team  at Crosshands Health Centre, Carmarthenshire.

The role will  involve working across the county, caring for patients living within the 3 localities of Carmarthenshire.

Working as part of the team, within Community Nursing services, you will be responsible for providing high quality care to patients in their own homes living with life limiting, palliative or terminal illnesses.

### Main duties of the job

The post holder will ensure high standards of individualised care are delivered to patients and their families and carers within the home environment, by following care plans and treatment interventions as delegated by the registered nurse, as well as having the ability to work competently without direct supervision.

Carmarthenshire Community Nursing Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current health policy.

This is a permanent position worked over a 7-day period including weekends and bank holidays.  Full time and part time hours available.

The post holder will need the ability to travel between sites in a timely manner.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

### Experience

**Essential**

- Experience of working within a relevant heath or social care environment, providing direct patient care

**Desirable**

- Experience of working with patient groups relevant to area of practice
- Experience of working within the NHS

### Language Skills

- Welsh Speaker (Level 1)

### Qualifications and Knowledge

**Essential**

- Level 3 qualification in a health care related subject Or equivalent experience and can evidence work based competency demonstrating theoretical knowledge, and the ability to undertake clinical care duties relevant for the role which is gained through experience and worked based training/short courses
- Knowledge of the Code of Conduct for Healthcare Support Workers in Wales and All Wales Delegation Guidelines (HEIW)
- Knowledge of relevant policies and procedures within a healthcare setting for example the principles of Safeguarding adults/children to keep patients within their care safe, and confidentiality for example General Data Protection Regulations (GDPR)
