You will be pivotal in ensuring people with a learning disability have their physical and mental health needs met in primary care services in a timely and appropriate manner. You will be able to work independently, supporting the delivery of person-centred care plans with a focus on prevention of ill health through promotion of healthy lifestyles. Communication and engagement are key for people with learning disability, so these are key skills required along with a positive and creative approach. You will be able to demonstrate that you are able to apply your skills to a wide range of situations, working with a range of people with a learning disability in the community. The team offers: - Consultation, advice, brief assessment and formulation to GPs on the needs of adults with learning disabilities who may also have associated physical health conditions, behavioural difficulties and/or mental illness. Advice and support to individuals and families about health needs. Support to develop accurate Learning Disability registers, attendance at annual health checks, and health action planning for people aged 14 and over with learning disabilities. Signposting to access the use of health and social care services and other organisations.
