The post holder will be required to have access to independent means of transport for work purposes to travel across Warwickshire county and to other agencies. Promoting Health and Preventing Ill Health Support CYP to improve and maintain their mental, physical, behavioural health and wellbeing where possible dependent on the trajectory of the CYP’s condition. Active involvement in the prevention of and protection against disease and ill health Engage in public health, community development, and in the reduction of health inequalities Support health and social care colleagues with completing assessment of CYP. Provide and Monitor Care Provide compassionate, safe and effective care and support to CYP in a range of care settings. Undertake training and complete competencies for specific nursing skills in order to facilitate care provision and assessment.
