The role will predominately be aligned to the East of Berkshire but cross cover as pressure and need arises. You will support individuals with Learning Disabilities, Autism and Complex needs to achieve improved health and wellbeing outcomes and greater independence within the Community. You will be an enthusiastic, suitably skilled and experienced health care professional to undertake this role. You will support the ongoing development of our community-based specialist health services, for people with learning disabilities across Berkshire aimed at supporting people to remain in their communities and minimising admissions. A commitment to supporting effective teamwork, with an excellent value base will be crucial to your success in this role. Work with us to ensure: - the best care in the right place for people with learning disabilities ### Main duties of the job - Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual.
