We are a multidisciplinary team of Specialist Nursing, Physiotherapists, Occupational Therapists and Pharmacy working closely to assess and promote wellness and independence of the population of Chorley, Preston and South Ribble that are presenting with deteriorating frailty symptoms.

Your role will be to support the Occupational Therapist and Physiotherapists within this team including undertaking assessments and carrying out treatment plans that fall within the scope of your skills and knowledge and under the direction of the therapists.

We are looking for a flexible candidate who is able to work across a range of settings and has experience working to support Occupational Therapy and Physiotherapy interventions within a community setting, with experience of assessment and provision of equipment according to patients needs.

The role involves working in patients’ own homes or with patients in a clinic environment. You will require the ability to travel to carry out home visits.

We are looking for a Technical Instructor/Assistant Practitioner with excellent communication skills, a natural problem solver and someone who has the ability to work autonomously as well as part of a team

- To contribute to service development, clinical audit and research and development in the arena of Community therapy assessment and intervention with a particular emphasis on high risk clients who would benefit from functional rehabilitation assessment e.g. Falls, decompensating frailty, advice around optimising occupation and independence.

### Knowledge

- Has knowledge of frailty/long term conditions.
- Have knowledge of mental illness and disability.
- Have knowledge of the cultures and religions of the area communities and to work across those cultures.

### Experience

- Experience of rehabilitation in the community
- Demonstrates the ability to work with minimum supervision.
- Is able to organise own time and workload.

### Skills and Abilities

**Essential**

- Proven problem solving and clinical reasoning skills.
- Able to work independently
- Demonstrates good written and verbal communication skills.
- Able to adapt communication style to suit different people, including use of verbal and non-verbal communication.
- Be physically capable of carrying out the duties required, including the manual handling of equipment or household items.

### Education/ Qualifications

**Essential**

- Assistant Practitioner Qualification or equivalent
- NVQ level 3 or equivalent
- Evidence of continued professional development
