Are you an experienced Registered Nurse Prescriber looking for your next challenge?

The Urgent Community Response Service (UCRS) at ESNEFT is seeking a highly motivated and skilled nurse to join our dynamic and innovative team. This is an exciting opportunity to deliver high-quality urgent care in patients' homes and care home settings, helping people receive the right care at the right time while avoiding unnecessary hospital admissions.

Working within our multidisciplinary team, you will use your advanced clinical skills to assess, diagnose, prescribe, treat and manage patients with a wide range of urgent healthcare needs. No two days are the same, making this an ideal role for an experienced clinician who thrives on autonomy, clinical decision-making and providing outstanding patient-centred care.

### Main duties of the job

As a Nurse Practitioner within UCRS, you will:

- Undertake rapid response assessments within two hours of referral.
- Assess, diagnose and manage urgent and complex clinical presentations in community settings.
- Independently prescribe appropriate treatments and interventions.
- Deliver short-term, intensive support to prevent hospital admission and facilitate recovery at home.
- Manage patients with frailty, multiple long-term conditions, acute deterioration and palliative or end-of-life care needs.
- Provide expert clinical advice and support to patients, carers and healthcare professionals.
- Work collaboratively with GPs, therapists, community teams, social care and acute services.
- Contribute to service development, quality improvement and clinical leadership within the team.

A car driver is essential.

### Knowledge

**Essential**

- Sound knowledge of a broad range of assessments including specialist relevant to the specialist care group and the relative benefits of each.
- Excellent knowledge of a range of advanced therapeutic interventions relevant to the care group including the relative benefits of each and the comparative outcomes.
- Excellent knowledge of the current evidence-based practice.
- Awareness of national guidance relevant to own specialities, including, consent issues, and other legal frameworks relating to care group

**Desirable**

- Awareness of ESNEFT strategic direction

### Experience

**Essential**

- Proven experience working with relevant care group
- Competencies relevant to care group
- Experience of managing complex and/or highly specialist cases

**Desirable**

- Experience of managing and supervising the work of others

### Qualifications

**Essential**

- Registered nurse recorded on NMC register
- Professional knowledge and skills underpinned by theory and experience, acquired through degree supplemented by specialist training, experience in specialist field
- Evidence of ongoing CPD
- Non-medical prescriber

### Skills/abilities

**Essential**

- Work as part of an integrated team
- Supervise the delivery of care to patients presenting with a range of urgent care conditions
- Good communication skills both written and verbal
- Ability to manage a team
- Work independently using own initiative
- Use Word, PowerPoint, email, excel and access (or similar spreadsheet or database packages)
- Car owner/driver/access to a motor vehicle (role will involve travel to other sites that cannot be easily accessed by public transport means)
