We have an exciting opportunity for an experienced Band 6 Nurse Coordinator to join ourSingle Point of Access Team. If you are experienced, enthusiastic, motivated and compassionate we would love to hear from you.

The service is open seven days a week, with various shifts to cover the service from 07:45 - 20:15.

### Main duties of the job

We cover the whole of the West Cheshire geographical footprint and the GP surgeries within it. Along with referrals from GPs we also work in partnership with North West Ambulance Service and we are part of the West Cheshire Health Collaborative.

As a Nurse Coordinator the successful candidate will undertake duties involving the triage of patients via NWAS and CAS 111 with the focus being on hospital avoidence were possible utilising the many community services and pathways that are avalible to SPA. You will also facilitate GP referrals to enable patients to get the right care at the right time and the right place.

## Person Specification

### Experience

**Essential**

- A Registered Nurse Adult, currently practising with post registration experience within a community/primary care setting
- Previous experience of telephone triage
- Leadership / management skills
- IT literate with good keyboard skills

**Desirable**

- Experience of face to-face triage / multidisciplinary / interagency working/and clinical streaming

### Qualifications

**Essential**

- Registered General Nurse (RGN)

**Desirable**

- Post –registration qualification in relevant subject
- Previous experience of facilitating clinical supervision

### Knowledge and Expertise

**Essential**

- Extensive current clinical knowledge
- Evidence of ongoing professional development
- Excellent communication skills, verbal and written
