\<p\>We are inviting applications for a motivated and skilled \<strong class="Yjhzub"\>Nurse Associate\</strong\> to join our dedicated community services team based at \<strong\>Chippenham\</strong\>\<strong\> Community Hospital\</strong\>. This is a full-time position offering a fantastic opportunity to provide high-standard, holistic, person-centred nursing care within the local community.\</p\>\<p\>About the Role:\</p\>\<p\>As a key member of our multidisciplinary team, you will contribute significantly to the assessment, planning, delivery, and evaluation of nursing care for patients on the district nursing caseload.
