Background

The Department of Health (NI) together with the British Medical Association, the Department of Health and Social Care (England), as well as the Department of Health (Wales) have been working together since February 2020 to formally negotiate a revised Specialty Doctor grade and introduce a new Specialist Grade.

In January 2021, an agreement was reached for a reformed specialty doctor contract and a new specialist contract.

These new contractual arrangements came into effect from 1 April 2021. The Specialty Doctor 2008 contract closed to new entrants on 31 March 2021 and all new Speciality Doctor appointments are to the 2021 Specialty Doctor and Specialist contracts.

### JOB SUMMARY

This Specialty Doctor post will provide support to Clinical Haematology at Antrim Hospital. The post holder will work closely with the relevant consultant staff to provide care to haematology outpatients and inpatients. The post holder will provide clinical haematology cover for Laurel House Chemotherapy Day Unit (Antrim) and assess inpatient referrals. The post holder will provide support for Haematology Clinics in Causeway Hospital as required. The post holder will work under the supervision of the consultant haematologists, Dr Scott McCloskey, Dr Chris McAuley, Dr Gavin Brown, Dr Philip Windrum, Dr Brídgín Merron and Dr Suzanne McPherson.

### KEY RESULT AREAS / MAIN RESPONSIBILITIES

The appointee will work closely with Consultant Haematologists to provide inpatient care for haematology patients. The appointee will attend Haematology outpatient clinics (OPC).

The appointee will provide medical cover for haematology patients attending Laurel House Chemotherapy Day Unit. This will involve assessment and admission of patients with complications of chemotherapy and performance of various day procedures including, lumbar puncture and bone marrow biopsy.

The appointee will provide support to junior medical staff in the care of haematology patients in the medical area.

The appointee will actively pursue continuing professional development \[CPD\] by attending postgraduate teaching sessions and other educational activities.

The appointee will contribute to the various teaching (undergraduate and postgraduate) and audit activities undertaken within the Medical Specialties Directorate.

The appointee will also take part in processes developed in the implementation of Clinical Governance.

To contribute to the setting and implementation of standards/guidelines/procedures for practice as required.

Comply with good practice in prevention of HCAI including Adherence to Trust policies including Hand Hygiene.

### The Area

Antrim hospital is part of the NHSCT comprising Antrim, Mid-Ulster, Whiteabbey, Braid Valley, Moyle, Causeway, Holywell, Robinson Memorial and Dalriada Hospitals

Antrim Area Hospital is a general hospital, commissioned in 1994, to provide a wide range of inpatient and outpatient services. It is the main acute hospital serving the NHSCT area and is located in a green field site adjacent to the M2 motorway and within easy reach of Belfast, Ballymena, Antrim, Magherafelt and the surrounding countryside.

At present there are 6 Consultant Haematologists based in Antrim Hospital, with outpatient clinics being held on other peripheral sites.

Current workload figures show a total of approximately 15,000 outpatient clinic attendances throughout the Trust each year, with approximately 430 inpatient episodes.

The Haematology inpatient service is based in Ward C7, Antrim Hospital. This 24 bed ward is shared with Renal Medicine and Rheumatology. There are rotating junior medical staff attached to Ward C7 who provide routine and emergency cover for haematology inpatients. Chemotherapy trained nurses administer inpatient chemotherapy treatment on C7.

The appointed Specialty Doctor in haematology will be working with Consultant Haematologists and Haematology Clinical Nurse Specialists at outpatient clinics and providing inpatient care. This Specialty Doctor will also be a member of the Haematology MDM.

Emergency haematology admissions are arranged through the Medical Admissions Unit Antrim Hospital, and are initially under the care of the Consultant Physician before being handed over to a Consultant Haematologist. Elective admissions are organised via the bed-management system. Where possible patients are admitted directly to C7.

Haematology Clinics are held in Laurel House where there is a chemotherapy day ward. This purpose-built facility was opened in 2005 and also provides Oncology clinics (for Lung, Gastro-Intestinal and Breast tumours in particular)

A designated Haematology/Oncology satellite pharmacy is located within the Chemotherapy Day Ward and provides a comprehensive service including reconstitution of intravenous cytotoxic and biologic agents and issue of oral cytotoxic and chemotherapeutic drugs.

An automated FBP analyser provides quick access to blood results.

The Chemotherapy Unit is staffed by 8 WTE chemotherapy trained nurses and managed by a Band 7 nursing sister. The unit is supported by 3 WTE PICC line nurses.

Monday, Thursday and Friday are the main chemotherapy treatment days for haematology patients. General procedures (bone marrow biopsies, blood transfusions etc.) are usually performed on Wednesday and Friday. There are general haematology clinics held in Antrim Hospital and satellite hospitals throughout the trust throughout the week.

The haematology medical team and associated admin staff are based in Antrim laboratory. In addition to haematology, pathology services on site include clinical chemistry, microbiology and cellular molecular pathology. Northern Trust pathology services hold full UKAS accreditation for all disciplines provided on both Antrim and Causeway Hospital sites.

There are currently a number of major ICT projects ongoing within the Trust and wider HSC which impact on pathology services. This includes the implementation of a new NI Pathology Information System (NIPIMS), expansion of NIPACs (picture archiving system) and the roll out of Encompass. Encompass is an HSCNI wide initiative which will support the long term sustainable digital transformation of health and social care records in Northern Ireland. Encompass will enable everyone involved with a person’s care to work from a single health and care record. Go live date for the Northern Trust is 7th November 2024.

Northern Ireland pathology services are also subject to review under the Blueprint Programme which

is exploring options for moving the management of laboratory services from individual Trusts into a

single regional pathology management structure, along with the Northern Ireland Blood Transfusion

Services (NIBTS) and the functions of the NI Pathology Network. It is not anticipated that medical

staff involved in front line patient care will be directly impacted on by this proposal.

### Continuing Medical Education

There are excellent facilities for study, lectures etc. in the Antrim Hospital Post-Graduate Medical Library.

A comprehensive library containing up-to-date reference books, periodicals and journals are provided in the Antrim Hospital Post-Graduate Medical Library. Computer facilities including Microsoft Excel;

PowerPoint and Word packages and CD Roms are available for Use by Medical Staff. Access to the Internet and databases Such as CINAHL, COCHRANE and MEDLINE are available. Books may also be requested from the Library at Queen’s, University, Belfast via the Queen’s catalogue.

Study leave is available by application to the Lead Consultant and all staff are encouraged to attend appropriate courses and make full use of study leave.

### Supervision Arrangements

Working at middle grade level. Full consultant supervision is available on site during working day.
