### JOB DESCRIPTION

### JOB SUMMARY

The NHSCT Clinical Fellows are exciting posts designed to allow Doctors temporarily outside of

training to gain experience of their preferred specialty whilst expanding and enhancing their CV

and competitiveness.

### KEY RESULT AREAS / MAIN RESPONSIBILITIES

This is a non-postgraduate training role, which will strengthen our operational Emergency

Department team. The purpose of this post is to support Emergency Department clinical work

alongside senior medical staff. It will also allow the successful candidate to pursue an interest in

Medical Education, Simulation and Quality Improvement.

The post forms a core component of the medical staffing. The successful applicants will join a

team of Consultants, Specialty Doctors, Resident Doctors and nursing specialist practitioners to deliver high quality services to Emergency Department patients. They will be responsible for the initial assessment of new attenders and re-attending patients, and will initiate investigations and plan and deliver appropriate care for these patients. They will also be required to liaise with specialty teams concerning patients who require specialty input or admission and for effective liaison and seamless medical care for such patients.

The post holder will be given similar training and educational opportunities as those in a training

post position equivalent to their previous experience. The post holder will be allocated an educational supervisor and will be given the opportunity to perform audits to enhance their CV. The post may also be useful for people working towards article 14 in the longer term or to progress onto substantive posts in the Trust.

### PROFILE OF HOSPITAL

The Northern Trust was established in April 2007 following the reconfiguration of services under the Review of Public Administration. With an operating budget of £548 million and approximately 12,000 employees, the Trust is responsible for the provision of responsive, safe, cost effective and efficient integrated health and social care services to a population of over 443,000 within the local council areas of Antrim, Ballymena, Ballymoney, Carrickfergus, Coleraine, Cookstown, Larne, Magherafelt, Moyle and Newtownabbey.

Causeway Hospital is one of two acute hospitals in the Northern Health and Social Care Trust offering 24 hour, 7 day a week acute services.

Other hospitals within the Northern Health and Social Care Trust include Antrim Hospital (40 miles from Causeway Hospital) which also provides a 24 hour, 7 day a week acute services. Whiteabbey Hospital, Newtownabbey and Mid-Ulster Hospital, Magherafelt provide subacute rehabilitation, intermediate care and a range of outpatient and day patient services but do not offer 24 hour acute services.

Altnagelvin Hospital, (Western Health and Social Care Trust) is 30 miles from Causeway Hospital and local elderly residents requiring fracture rehabilitation are transferred to Causeway Hospital for this purpose.

Clinical networks with both Causeway Hospital and Altnagelvin Hospital have been developed and it is envisaged that further networks will develop in other specialties

### THE AREA

Causeway Hospital serves a population of 100,000 plus a large tourist population during holiday times, particularly summer months. There are many sporting and festival events during the summer which attract large crowds. The town also accommodates a substantial student population.

The largest town in the Causeway Locality is Coleraine, which is approximately 55 miles from Belfast. The locality includes a large coastal area stretching from Castlerock to Cushendall and reaches inland to the country towns of Ballymoney, Garvagh, Kilrea and Rasharkin. Belfast International Airport is one hour by car.

The University of Ulster is situated on the outskirts of Coleraine and there is good liaison between the University and Causeway Medical staff. There is potential for further joint research.

The area has a high standard of residential accommodation. There is a wide selection of schools with excellent academic records. The hospital is on the Antrim Coast, an area of outstanding natural beauty. There are excellent recreational facilities including many blue flag beaches, restaurants, surfing, boat trips and internationally famous golf courses. All are a short drive from the hospital.

### HOSPITAL SERVICES

Causeway Hospital opened in May 2001. It is a modern hospital, of low-rise design on a large landscaped site.

There are 240 in-patient beds and a dedicated Day Procedure Unit with excellent out-patient facilities with well established medical and specialist medicine clinics.

o Full accredited (CPA – UK Ltd) diagnostic laboratory

o CT Scanner with 24 hour Consultant Radiologist cover.

o On site MRI scanner

o Carotid Doppler assessment service for TIA/stroke

o Excellent non-invasive cardiology investigative services

o Pulmonary function laboratory on site.

An X-ray PACs system enables viewing of X-rays on any computer terminal throughout the hospital with Boneview AI Xray enhancement

### Bed Complement

o General Medicine including Acute Medicine, Respiratory Medicine Gastroenterology, Diabetes and Endocrine, – 37 beds

o Coronary Care/Medical Monitoring Unit – 14 beds

o ICU/High Dependency – 3 ICU beds/2 ICU and

2 HD beds

o acute elderly care including acute stroke Unit – 36 beds

o Medical Admission Unit – 12 beds (+5 overflow beds)

o Acute ambulatory unit

o Ambulatory surgical unit.

o General Surgery (including Paediatric Surgery and Urology)

o Day Procedure Unit

o Maternity services /Gynaecology

o Acute Psychiatry

### Medical Investigation Unit staffed by Clinical Physiologists which provides

o Non invasive Cardiac investigations – echocardiograms, Exercise Stress Tests and Holter monitoring.

o Gastroenterological Investigations

o Pulmonary Function Assessment

Endoscopy Facilities in the Day Procedure Unit

o Fibreoptic Bronchoscopy

o Gastroscopy

### o ERCP

o Colonoscopy

o Cystoscopy

Cardiology emergencies are admitted continuously to the on-site CCU. There is immediate access to PCI via Altnagelvin Hospital.

There is a social work team on site and a range of Allied Health Professional and support staff.

The Resuscitation Training Officer provides ALS courses and on-going training in Basic and Immediate Life Support.

### WORK OF THE EMERGENCY DEPARTMENT

Attendances – 55,000 per year with an undifferentiated case mix which include Trauma, Paediatrics, General Medicine, General Surgery, Obstetrics & Gynaecology and Psychiatry. The department is the setting for acute stroke lysis.

The department liaises with Altnagelvin and Antrim hospitals in the management of acute STEMI, Trauma and Orthopaedics, ENT and Ophthalmology presentations.

The department is a designated major trauma unit in the regional trauma plan.

### FACILITIES

9 Bedded fully monitored major’s area

4 Rooms for minor injuries including dedicated eyes/ENT room

Separate paediatric area

A dedicated ENP led minor injuries area with self-select model

Ambulatory care area.

4 Bedded Resuscitation area

24 hour access to acute mental health provision via the RAID team.

The department has patient tracking systems (Encompass)

Complimentary clinical services include Acute Medicine working partially out of the ED and a consultant led front door frailty service. The department has access to a wide range of inpatient and outpatient diagnostic services and multi-disciplinary teams.

There are well developed plans included in the NHSCT to significantly expand the footprint of the department.

### MEDICAL STAFF COMPLEMENT

8 Consultants posts on a 1 in 8 weekend rota

8 specialty doctor posts

### 1 GPST1

14-person junior medical staff rota

5 ENP providing 12 hour 7 day a week cover.

Nursing Staff – 45.5 WTE all bands

Undertake assessment, diagnosis and treatment, including appropriate investigations, monitoring, etc and follow-up arrangements to ensure a high standard of clinical management within the Emergency Department and the Observation Unit.

Ensure continuing responsibility for patients in his/her charge and undertaking appropriate administrative duties

Contribute to the multi-disciplinary assessment and treatment functions and provide clear and timely communication to patients, carers, General Medical Practitioners, Allied Health Professionals, Community and Social Services staff as appropriate to the needs of the patient

Ensure safe handover of patients with colleagues

Co-ordinate leave plans of annual/study/professional/examination leave with colleagues to ensure safe level of cover and adhere to the Trust Leave policy in respect of period of notice of leave

Ensure that all sick leave is notified to the appropriate officer in the Trust in accordance with the Managing Sickness Absence Policy

Develop departmental protocols in conjunction with other clinicians and professionals within the Trust and with other health care providers including as a result of audit, national guidelines, etc

The post is full-time and will provide 24/7 cover. Weekend and nightshift cover will be required as part of the post. Eight (8) hours a week are dedicated to agreed continuing professional activities.

### SALARY

Speciality Registrar - £46,274 - £70,914 per annum (pro-rata for part-time)

### HOURS

Full time hours as per sample rota, exclusive of meal times. Working as part of a band 1A rota.

### PAY PROTECTION
