### Job summary

37.5 hours per week - permanent.

The CNRT deliver patient-centred MDT neuro-rehabilitation to patients with neurological diagnosis across Plymouth, South Hams and West Devon. We are driven to help patients maintain their independence and quality of life by delivery patient goal-based rehabilitation programmes.

You will contribute in the delivery of rehabilitation under guidance from registered professionals to support patients achievement. This will be achieved through implementing a variety of therapeutic interventions and practise of activity of daily living depending on patient's needs.

You will provide support and supervision in all aspects of rehabilitation to patients. Rehab is delivered in a variety of settings including CNRT patient homes, outpatients and group classes at Mt. Gould Hospital.

Interviews will be conducted face-to-face due to the practical element of the assessment. Please contact us if you require any reasonable adjustments to support your attendance.

Follow care plans from registered staff for occupational therapy, physiotherapy, speech and language therapy and nursing interventions to support the rehabilitation of patients.

### Job responsibilities

### Knowledge

**Essential**

- Clear evidence of understanding of neuro rehabilitation process and services.
- Clear evidence of understanding the roles multi-disciplinary team.
- Understanding of own role boundaries.

### Experience

**Essential**

- Clear evidence of experience of formulating therapeutic relationships (Correlates with employment history).
- Previous experience of working within a healthcare setting (employment, work experience, volunteering)

### Qualifications

**Essential**

- Recognised level 3 qualifications or demonstrated ability and commitment to achieve a relevant level 3 qualification.
- Level 2 functional skills, or as a level 3 apprenticeship such as the
- Senior Healthcare Support Worker apprenticeship (or other level 3 qualification pathway).

- Can evidence effective caseload management.
