An exciting opportunity has arisen for a motivated, compassionate and dynamic Band 5 Nurse to join our expanding Neonatal Community Outreach Team (NCOT) at The Shrewsbury and Telford Hospital NHS Trust.

Working with the NCOT Lead Nurse, Senior NCOT Nurses and multidisciplinary team, you will provide high-quality, family-centred neonatal care, supporting babies and families during their transition from hospital to home.

The NCOT service supports early discharge and ongoing care of babies requiring interventions such as oxygen therapy, phototherapy, nasogastric tube (NGT) feeding, home phototherapy, medication administration, growth monitoring and developmental support. The team also cares for babies discharged weighing less than 1.8kg, those born below 35 weeks corrected gestational age, and babies with additional medical or nursing needs requiring specialist community follow-up.

Working across both neonatal inpatient and community settings, you will maintain and develop neonatal skills while promoting continuity of care. The service covers Telford, Shropshire and Powys and works closely with neonatal, maternity, paediatric, health visiting and primary care teams to provide safe, seamless and effective care for babies and their families.

The post holder will work across both neonatal inpatient services and the community setting to maintain and develop neonatal clinical skills while supporting continuity of care for families.

### Main duties of the job

The post holder will assess the suitability of referrals and ensure eligibility criteria is met. They will be involved with the discharge process to ensure a seamless, timely transfer from hospital to the home environment and communicate with families to prepare them for the transition home.

The post holder will risk assess patient needs, environment and care required during each visit.

The post holder will need recent Neonatal experience within a hospital setting to ensure they have the clinical knowledge to identify deterioration or recognise improvement of the patient and adapt care plans accordingly.

As an autonomous practitioner they must have the ability to solve problems, confidently function in a lone worker capacity, effectively manage time and demonstrate excellent communication skills.

The post holder will practice in a holistic, non-judgmental, caring and compassionate manner that avoids assumptions, supports social inclusion and respects individual diversity. They will work in partnership with the MDT, service users, their carers and families. The post holder will contribute to the practice development within their clinical care setting and work in accordance with Trust Policies, Procedures and Guidelines and will be responsible for completing any work-based training associated with the role.
