This is a pivotal role focused on delivering high-quality, coordinated care for people living with MND and supporting their families across community services. You will act as a key point of contact, ensuring seamless care pathways, improved patient experience, and timely access to multidisciplinary support. This is an exciting opportunity to play a crucial role in the care coordination of patients with MND. This post will be primarily based in the west of the county but will work closely with colleagues in the east to support providing equitable care across the county.
