This is a fixed term post for 12 months covering maternity leave, supported by a Motor Neurone Disease Association (MNDA) grant. Working within a nationally recognised MND Care Centre, you will contribute to the coordinated, proactive and multidisciplinary care model recommended in NICE NG42. The role focuses on collaboration, working closely with the MND Clinical Nurse Specialist, Advanced Nurse Practitioner, Consultant Nurse, Neurologists and the wider MDT to ensure seamless, person centred support from diagnosis through to ongoing management. You must have an understanding of the needs of patients with a life limiting illness; possess excellent communication skills for advising and supporting patients, relatives and other health professionals along with a willingness to learn about Motor Neurone Disease (MND) We welcome applications from experienced Nurses or Therapists who demonstrate advanced communication skills, sound clinical reasoning and a commitment to teamwork and service improvement. You will join a supportive, forward thinking team dedicated to improving outcomes and experiences for people living with MND. This is a full time, Monday to Friday role with no weekend or bank holiday working an opportunity to make a meaningful difference within a collaborative, specialist environment ### Main duties of the job The Band 7 MND Care Centre Coordinator will hold an appropriate professional qualification (RN or AHP) and work collaboratively within the MND team and wider multidisciplinary network to deliver coordinated, proactive and person centred care in line with NICE NG42 guidance.
