### JOB SUMMARY

To submit robust timely data to the MINAP data set on all patients that present to the Trust with acute coronary syndromes.

To work as part of the cardiology team to carry out a programme of ongoing data collection and audits within Cardiology Services in Northern Ireland from first access to service to patient discharge.

To work in conjunction with Trust / Cardiac Network / HSCB in the initiation and participation of audit and research in order to ensure the development of evidence based pathways of care in relation to diagnosis and management of patients with cardiac disease.

To work in conjunction with the Trust management team to ensure the strategic aims of the Trust / Cardiac Network / HSCB are achieved and the standards of the service meet the performance indicators and raise awareness of areas of concern.

### KEY DUTIES / RESPONSIBILITIES

To submit timely high quality data to the MINAP data set for all patients presenting to the Trust with troponin positive chest pain.

To ensure the maintenance of a high standard of data capture and information through continual evaluation and analysis of activity and outcomes.

Discussed emerging information with individual clinical teams responsible for the care in order to critically evaluate the appropriateness on an individual and Trust basis

Sharing of good practices throughout the Trust area.

Assessing and analysis any gaps in the clinical pathway from first access to services to discharge from hospital across all components including primary care, NIAS, A&E and Cardiology Departments. Systems may also vary across legacy Trusts.

Work with the clinical teams responsible for care and help facilitate the subsequent service improvement required as the result of the analysis.

Planning the data collection across the Legacy hospitals and coordinate with the various clinical teams / professionals involved in the care, including those working in other organisations.

To assisting in the collation and analysis of other identified data at local level in order to ensure the completeness and the clinical quality of the information prior to sharing with other agencies/Trusts.

To work with clinical teams to ensure that chest pain patient assessment and treatment is developed in line with the current guidelines and standards.

To complete relevant documentation and maintain accurate database for patients seen.

Undertakes surveys or audits necessary to own work and participate in R & D and clinical trials appropriate to the role.

To Establish and maintain communication with people across the Trust sites, NIAS and Network to ensure appropriateness and timely provision of care.

To communicate project objectives, effecting change and improvements in care based on project findings to managerial level at all Legacy Hospitals.

To prepare, present and deliver review reports at all levels internal and external to the Trusts.

To identify areas of service redesign / improvement based on audit findings and discuss with relevant personnel both internal and external to the Trust as appropriate.

To contribute to the development of the improvement programme policies and practices working with multiprofessional team members and other improvement programme teams and agencies, as required.

Identifying data requirements and introducing systems to ensure collection of robust and clinically correct data is available for audit purposes.

To providing clinical expertise of ward and other staff in order to inform, teach and benefit patients.

To organise and provide training programmes in accordance with the identified needs for the department as required.

To assist with the in-house training of nursing staff with regard to specific aspects of cardiology patient pathways.

### CLINICAL AND PRACTICE RESPONSIBILITIES

1.1 Work both independently and in collaboration with the multi-professional

team, including medical colleagues.

1.2 Ensure patients are managed within recognized and agreed national and regional nursing guidelines.

1.3 Be proactive in developing flexible and innovative approaches to care for patients following Myocardial Infarction.

1.4 Provide a continuing point of contact and act as an educational resource for patients, relatives/carers and staff within the Trust

1.5 Actively engage with patients and carers in goal setting and care planning ensuring that all care provided is patient centred.

### SERVICE IMPROVEMENT

2.1 Contribute to an environment which facilitates the implementation of proven research/evidence based practice.

2.2 Support the Cardiology team in the engagment with commissioners, staff from hospital settings, general practictioners, in re-designing services using appropriate quality improvement approaches.

2.3 Engage patients and/or carers in redesigning services and processes to support improved management for patients with Heart Disease

2.4 Ensure information provided for patients and their carers is accurate adheres to current guidelines, is timely and easily accessible and is in a format that enables easy application by all those involved in patient care.

2.5 Submit statistical information and professional reports accurately and promptly.

### 3. PERSONAL/PROFESSIONAL RESPONSIBILITIES

3.1 Build on and develop innovative and flexible approaches to care in primary and community care settings.

3.2 Actively contribute to developments within the Trust

3.3 Adhere to the Nursing and Midwifery Council’s guidelines, policies and ‘Code of Professional Conduct: Standards for Conduct, Performance and Ethics and be individually accountable for meeting the requirements of NMC revalidation.

3.4 Understand the role of the clinical supervisor within the Framework of Clinical Supervision and actively participate in clinical supervision groups / sessions

3.5 Maintain and update own professional knowledge and expertise within heart disease.

3.6 Provide effective clinical leadership to support and inspire colleagues to improve standards of care and promote continuous improvement in the management of Myocardial Infarction.

3.7 Act as a resource within the Trust as a specialist advisor

3.8 Encourage and facilitate the sharing of ideas and examples of good practice.

3.9 Network with other MINAP nurse and with other professional colleagues as appropriate.

3.10 Keep accurate records, submit statistical returns and compile reports as required in accordance with the requirements of clinical governance and NMC requirements.

3.11 Support the Lead Nurse to develop standards, policies and guidelines to improve own practice and that of others. Act as change lead for the implementation of clinical guidelines.

3.12 Participate in staff recruitment and development and induction programmes for newly appointed staff.

3.13 Attend all mandatory study days as agreed by Trust policy.

3.14 Attend and contribute to monthly cardiology Governance meetings, Morbidity and Mortality meetings and any other appropriate groups within the department.

3.15 Identify with Line Manager, the extended role skills required to lead and develop the service and undertake any necessary training required to attain competency in these extended roles.

### 4. EDUCATIONAL RESPONSIBILITIES

4.1 Work with others, including Clinical Governance Team(s) to set up a regular information, training and educational programme for all health professionals, including the dissemination and implementation of British Society Guidelines, such as those from European Society of Cardiology and from The National Institute for Care and Health Excellence (NICE) and have an extensive understanding of the evidence base of the pharmacological treatments available and the appropriate application of guidelines regarding device insertion.

4.3 Encourage involvement of the patient and their carers’ in decision making to improve the understanding of the Cardiovascular disease and its management, identifying their needs, providing education, support and monitoring.

4.4 Promote health education advice including referral to appropriate programmes, such as “managing your condition”
