Salford Home Based Treatment Team is a 24/7 service that supports individuals in crisis at home, as an alternative to psychiatric inpatient admission. As a key member of a multidisciplinary team, you will undertake comprehensive mental health assessments, develop collaborative care plans, provide evidence based interventions, undertake risk assessment and managment, and support service users and families during periods of significant distress. You will work closely with inpatient services, community teams, emergency services, and partner agencies to ensure safe, effective, and recovery focused care. We are looking for applicants who demonstrate excellent assessment and communication skills, sound clinical judgment and a commitment to delivering person-centred, recovery focused care.
