An exciting opportunity has arisen to join the Closer2Home Intensive Home Treatment Team, this post is brand new to the service, so we are looking for an enthusiastic and motivated individual to help shape and develop the role.

Closer2Home is a specialist intensive home treatment service that supports young people with acute mental health problems in their homes by providing intensive input and support using a bespoke package of care tailored to each young persons needs.

This is an exciting opportunity for a passionate clinician or newly qualified practitioner to develop their career and make a powerful impact on the lives of young people when they need it most. Hospital admission can be lifesaving but often has a detrimental effect on young peoples as well. Closer2Home provides a service that helps young people in crisis to remain in their own home, in their own community where they can get involved in meaningful activities that are worthwhile to them.

### Main duties of the job

This is an exciting opportunity for a Band 5 nurse to gain experience in a community alternative to tier4 admission service, designing and delivering time-limited programmes of intensive home treatment to young people who would otherwise have been admitted to an inpatient tier-4 unit.

The post holder will work under supervision of senior colleagues to develop skills to become an autonomous and accountable practitioner within the service, who can lead on a small caseload to the delivery of treatment in the community. This will involve the assessment, planning, implementation and evaluation of the time-limited programmes of intensive home treatment to young people who would otherwise have been admitted to an inpatient tier-4 unit.

This post will work collaboratively with multidisciplinary teams across Hampshire supporting young people and families in the community as an alternative to an inpatient admission.

Alongside the clinical aspect of the role the post holder will be involved in developing and sustaining partnership working with individuals, groups, communities and agencies, providing clinical supervision to trainees and other professionals and contributing to the development, monitoring and auditing of identified service improvements as appropriate.

The applicant will be expected to develop the skill to lead shifts together with support from senior clinicians.

The post holders working hours will be across a shift pattern which includes evening and weekend work.

### Qualifications

- Nationally recognised core qualification RN, RMN, RNLD.
