In this role, you will undertake comprehensive assessment, care planning, implementation and evaluation, using evidence-based formulations to develop person-centred packages of care that support recovery and reduce risk. You will communicate complex and sensitive information with empathy and clarity, working closely with service users, carers, colleagues and partner agencies to ensure coordinated, respectful and inclusive support. You will work within the Care Programme Approach (CPA), contribute to multidisciplinary and multi-agency working, and provide expert advice on service user needs. You will maintain accurate clinical records across electronic systems, including RiO, PETs and safeguarding systems, ensuring compliance with Trust policies and professional standards.
