This is a fantastic opportunity to be part of the Manchester Referral and Assessment Hub.

We are looking for skilled practitioners, with mental health experience, who are keen to be part of a new service in Manchester, that is central to our improved community mental health offer. You will be a practitioner who is able to complete triage and assessments at the front door service into secondary care mental health.

Successful candidates will play an important part in the in the delivery of high-quality service provision.

As part of the team, you will be empowered to shape and build the service as community transformation develops in Manchester. You will help build and maintain positive working relationships with colleagues and the wider network.

### Main duties of the job

As a practitioner within the RAH you will -

Provide clinical provision of rapid assessment and care/pathway planning for people referred into the RAH presenting with mental health problems.

The post holder will be expected to carry out mental health assessments, accessing all available sources of information and including, where appropriate, the views of family and carers when relevant.

To undertake and record comprehensive need/risk assessments, developing clear goals and timeframes for the onward interventions with the service users.

## Person Specification

### Knowledge

**Essential**

- Knowledge of the wider community system
- Independently undertake initial mental health assessments identifying a service users’ needs and plan goal focused onward care.
- Complete risk assessments which reflect this risk a service user may pose.
- Transfer the risk assessment into robust risk management and pathway planning.
- Effective communication with service users, carers and members of the multidisciplinary teams.

**Desirable**

- Post registration experience of working in mental health community services.
- Have a working knowledge of primary care and secondary care services

### Experience

**Essential**

- Experience of undertaking initial biopsychosocial assessments
- Evidence of post registration training/development.

**Desirable**

- Post registration experience of working in mental health community services

### Education/Qualification

**Essential**

- One of the following Professional Registrations:
- Registered Mental Health Nurse
- Registered Social Worker
- Registered Occupational Therapist

**Desirable**

- NMP

### Work related circumstances

**Essential**

- Ability to work flexibly over a 7 day period between the service
- Ability to travel to a variety of community locations within the Division
