We are a small team and work in a rural catchment area. The role will take on the care co-ordination for clients and provide leadership to support to others, to ensure that full assessment of individual care needs are undertaken, that care is planned, implemented and reviewed on a regular basis. Dialog + is supporting patients care plans and recovery goals which you will manage for patients on your caseload. You will participate on the duty rota and be a member of the MDT. ### Main duties of the job - To participate on the duty rota. - To act as a role model and promote effective teamwork across the service to meet individual client needs. - To ensure that the CPA and Mental Health Act is adhered to at all times, and to work within the boundaries set out by the Trust’ Policies and Procedures. Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.
