This role also involves working close with our community partners such as GP practice to ensure we can provide the best community placed base care. The role is key to dissolving the barriers that can exist between mental health and physical health, social care, voluntary and community social enterprise (VCSE) organisations and local communities, and primary and secondary care, to deliver integrated, personalised, place-based and well-coordinated care for adults and older adults ### Main duties of the job We are seeking applications from Registered Mental Health Nurses.
