We are an enthusiastic and a welcoming team who hold quality patient care at the heart of what we do.  We have a team approach in the delivery of care and work proactively in order to ensure the best outcomes for our patients.  We understand the importance of staff development and will support professional growth. ### Main duties of the job For this post you will need to have excellent communication skills and the ability to work proactively in a multidisciplinary team environment.  You will be expected to support the team and conduct biopsychosocial assessments, establish and manage care plans for patients who present at Basildon Hospital.  As a Senior Mental Health Hospital Liaison Nurse, you will be expected to support STR workers, Associate Practitioners, Community Nurses, Advance Nurse Practitioners and the Clinical Lead in implementing effective changes within the team to ensure the service meets the Trust's objectives. ### Detailed job description and main responsibilities The post holder should work well under the overall supervision of the Advance Nurse Practitioner and Team Manager.  You will be required to undertake the duties of the post and also deputise in the absence of the Clinical Lead. Active participation will be required in the training and education of learners allocated to the community settings, and will also encourage integration between the ward teams and the community services. The successful candidate should be able to demonstrate being fully IT literate. Support is given by way of regular meaningful supervision and appraisal, reflective practice and Trust wellbeing services of advice and counselling.  Continuous Professional Development is supported and encouraged.
